VOLUNTEER LIABILITY WAIVER

I, hereby agree that if I am accepted as a volunteer for Happy Cats Haven, Inc. (“HCH”), I agree to comply with all of the rules and regulations that may be established from time to time by HCH. I understand that failure to comply with the rules and regulations of HCH may result in my immediate termination as a volunteer.

I understand that a 2 hour per week, 3 month commitment is required of all volunteer positions.

I understand that if accepted as a volunteer, all services performed by me will be performed on a strictly volunteer basis, and that I will receive no remuneration, pay or compensation of any kind, that I will not be an employee of HCH nor otherwise derive any benefits normally available to employees of HCH, and that HCH shall incur no liability of any nature as a result of my volunteering for HCH.

I acknowledge that in handling animals and performing other volunteer tasks there exists a risk of injury, including physical harm or death, and that all services performed by me will be done at my own risk. Therefore, on behalf of myself, my heirs, and personal representatives, I hereby release, discharge, indemnify and hold harmless HCH and its assigns, successors, agents, staff, board of directors, employees, contractors, and representatives from any and all claims, causes of action, or demands of any nature or cause whatsoever, including costs and attorney fees, arising out of or relating to my volunteering with HCH, including but not limited to animal bites, accidents, or injuries.

I understand that public relations are an important part of volunteering with HCH. On behalf of myself, my heirs, and personal representatives, if accepted as a volunteer, I give HCH permission to use and publish photographs taken of me as a volunteer for use in its public relations efforts.

PARENTAL/GUARDIAN CONSENT FOR TEEN CAT CORPS (AGES 12-17):
Youths between the ages of 12 and 17 are eligible to volunteer with a parent, legal guardian or trusted family friend who is 18 years or older. We are sorry, but youths under the age of 12 cannot volunteer because of liability issues.

I, represent that I am the parent or legal guardian of the Teen Volunteer named , a minor, (the “Volunteer”), and by my signature below consent to and allow my on/daughter/ward to perform volunteer services for HCH under the terms and conditions set forth above for volunteers.
